The European Partnership on Transforming Health and Care Systems (THCS) has launched its third Joint Transnational Call (JTC) for proposals: “Better care closer to home: Enhancing Primary and Community Care.” This call aims to support research and innovation projects that strengthen primary and community health care systems, equipping policymakers with the knowledge and tools to navigate necessary transitions in these critical sectors.

With the participation of 34 funding agencies and an indicative budget exceeding €35 million, JTC 2025 represents a significant opportunity for organisations to contribute to reshaping health and care systems across Europe and partner countries.

Scope of the call

The JTC 2025 focuses on:

• Innovative solutions: Delivering financial, organisational, and practice-based service innovations that support system transformation.
• Knowledge sharing: Accelerating the exchange of best practices across countries and regions.
• Policy impact: Providing policymakers with evidence-based tools to manage transitions in primary and community care.

How to apply

Joint proposals must include two components:

1. A pre-proposal form outlining the planned work.
2. An electronic submission detailing partner information and financial plans.

Access the online submission platform. Additional resources, including the call text, guidance for applicants, and pre-proposal templates, are available to assist applicants in preparing submissions.

To facilitate partnerships, a Partner Search Tool is available, enabling organisations to post partner offers or requests and browse a comprehensive list of potential collaborators.
